Access New Patient Forms
Save time at your first visit by filling out your new patient forms online. Completing your forms ahead of time allows us to better understand your dental history, goals, and any concerns—so we can make the most of your time with us. It also helps our team prepare for your appointment and personalize your care from the start. As soon as you make your appointment with us and we create your basic profile that includes your full legal name, date of birth, phone number and email, you will be able to access all the forms needed. If you have any trouble accessing the forms, we can help you access them during your first visit with us.

Yes, we offer flexible in-house plans to fit your budget. Contact our office to explore your options and get the care you need on a timeline that works for you.
We are in-network with Delta Dental PPO + Premier Network. While we do not accept HMO, DMO, or DHMO plans, we can work with most PPO plans on an out-of-network basis if they reimburse based on UCR (Usual and Customary Rates). If the insurance company is out of network and does not allow Assignment of Benefits (sending payments directly to the office), you will be expected to pay in full and we will help you submit the claims for reimbursement. It is very important that you provide accurate details to help reduce errors and wait times. Have questions about your plan? We’re happy to help you navigate the details.
We make every effort to predict your out of pocket expenses based on the information you provide and what we can find out from your plan. It is important to understand that estimates may not always be accurate even when a Pre-Estimate is received. The total fees will always be reviewed with you in advance, and any changes will be discussed before rendering any treatments. Your final bill may be the result of any discrepancies from the original estimate and what the insurance ultimate pays. It is important that you become familiar with your insurance coverage even though we help utilize your insurance as a courtesy.
